Chronic Disease Prevention & Management

Through our Chronic Disease Prevention & Management focus area, we focus on two preventable and manageable chronic conditions: Type 2 diabetes and HIV/AIDS. Type 2 diabetes and HIV are an alarming concern in our community so we aim to support the prevention and management of these conditions.

The Challenge:

  • 9% of adults in Santa Clara County, and 11% of adults in San Benito County are diagnosed with diabetes. 
  • The prevalence of HIV in ages 13+ is rising in both counties. In Santa Clara County, HIV prevalence is 188.9 per 100,000 people and 87.5 per 100,000 people in San Benito County. Research also shows a growing concern for non-HIV sexually transmitted infections (STI). 
  • Research shows that these conditions primarily impact low-income communities of color. These communities are historically impacted by deep disparities, making them more susceptible to serious illness and even death.

Chronic Disease Prevention & Management Funding Priorities

Our Chronic Disease Prevention and Management focus area aims to prioritize improving health outcomes through robust support, including connection to clinical care and wrap-around services. We support organizations, programs, and projects working towards this goal through the following Funding Priorities:

  • Enhancing the infrastructure and capacity of clinical providers and community-based organizations that improve health outcomes for people with prediabetes, type 2 diabetes, and HIV/AIDS.
  • Supporting policies and programs that focus on health outcomes for people with prediabetes, type 2 diabetes, and HIV/AIDS.  
  • Increasing guideline-based STI/HIV screening and rapid connection to care for individuals of all ages.
